Caregiver Consultant Services
The CARE collaborative, a partnership among local clinics, hospitals and organizations, offers a service that provides caregivers with support, counseling, education, and service coordination via a Caregiver Consultant.
Caregivers are relatives, friends or neighbors that help others remain in their homes by assisting them with daily living tasks. Caregivers often find themselves needing services and support but are not sure how to find it. Caregiver Consultants specialize in finding that help and teaching caregivers how to use it.
Caregiver Consultants are available in area clinics. Consultations are also provided in-home, by telephone and email. They are trained to counsel caregivers by:
- Providing support
- Providing assistance by referring to available services
- Providing educational opportunities
- Facilitating support groups for caregivers
No referral is necessary to call or visit a Caregiver Consultant. Contact one of the following organizations in your area to arrange an appointment:
